TERMS AND CONDITIONS

1. Assumption of Risk & Release of Liability

I, the undersigned participant (or parent/legal guardian of the participant if under 18), understand that participation in football training sessions involves inherent risks, including but not limited to falls, collisions, muscle strains, sprains, fractures, concussions, and other serious injuries.

 

I voluntarily assume all risks associated with participation and release, discharge, and hold harmless LOZ Gridiron Academy, its coaches, staff, and facility partners from any liability, claims, demands, or legal actions arising from participation in these training sessions.

 

2. Medical Treatment Authorization

In the event of injury or medical emergency, I authorize the coaching staff to seek medical treatment for the participant if necessary. I understand that I am responsible for all medical costs incurred as a result of participation in this program.

 

3. Media Release (Photo & Video Consent)

I grant LOZ Gridiron Academy permission to capture and use photos/videos of the participant for marketing, promotional, and instructional purposes. This includes but is not limited to social media, website content, and training materials.

 

I understand that I will not receive compensation for the use of these images/videos and waive any claims related to privacy, publicity, or compensation.

 

4. No Refund Policy

I acknowledge that all payments are final and non-refundable. Missed sessions due to personal conflicts, injuries, or scheduling issues will not be refunded or credited unless canceled by LOZ Gridiron Academy.

 

5. Code of Conduct

I agree to ensure that the participant maintains a positive attitude, respect for coaches and fellow athletes, and follows all training rules. Disruptive or unsafe behavior may result in dismissal from the program without refund.
